Vet loses license for euthanizing the wrong dog.

Due to Covid restrictions vets were seeing patients in parking lots and sometimes seeing them without their owners present. It was not an ideal time. 
However in this case the negligence goes beyond the difficulties. In the death of Cooper, an eight year old husky in good health, the vet walked out to the parking lot. He did not confirm who the dog was. The owner thought he was about to take blood. Instead of drawing blood, he injected the dog with euthanasia drugs. The dog died immediately. At no time did he confirm that this was the dog he thought he was to euthanize. 

Before removing his license, the Nova Scotia Veterinary Medical  Association reviewed  Dr  Sietse  VanZwol's work over the last year before the incident.  They felt for the safety of future patients, he should no longer practice veterinary medicine.

I think there were signs that the vet shouldn't have been practicing medicine before the incident with Cooper. It sounds like his work was not up to proper standards long before this incident. It is good that he has lost his license and must loss. For the person who was with Cooper and watched him die, that moment will haunt her forever. 

Someone must have already noticed the vet's work was not up to standard. Why was he not reported or at least watched?

There were plenty of signs that he was an incompetent vet.

More than one animal died because of his mistakes.
